After You Book
Once your appointment is scheduled, you will receive two emails:

From our office: a Functional Medicine Intake Form

From the Jane App: access to your Acupuncture Intake Form, Consent, and Disclosure documents
(These forms can be completed directly within the Jane App or patient portal.)
Before Your App
Please complete all required forms prior to your visit.

Lab Results ( If Available )
If you have recent lab test results, you may bring printed copies, or I can securely review them through your patient portal during your visit.

How to Prepare for Your First Visit
1. Eat a Light Meal Before Your Appointment
A small meal 1–2 hours before your session helps maintain stable energy.
2. Arrive Well-Hydrated
Hydration supports circulation and enhances your response to treatment.
3. Wear Comfortable, Loose Clothing
Loose sleeves or pants make it easier to access acupuncture points.
4. Avoid Caffeine Right Before Your Visit
Caffeine can elevate anxiety and affect pulse assessment.
5. Limit Heavy Exercise Before Your Session
Arriving calmly allows for a more accurate evaluation of your symptoms.
6. Download and Complete Your Patient Forms
Please complete your forms in advance to ensure a smooth start to your visit.
7. Bring Any Recent Lab Results
Lab work can support the Functional Medicine portion of your evaluation.
8. Avoid Strong Fragrances
Please avoid perfume or scented lotions, as some patients are sensitive to scents.
9. Arrive a Few Minutes Early
This allows time to settle in and begin your session relaxed.
10. Bring a List of Your Top Concerns
Listing your main symptoms or goals helps guide your personalized treatment strategy.
Managing chronic diseases often requires multiple visits, which can be expensive for some individuals. To address this, I offer affordable, small-group workshops on Saturdays focused on specific conditions. These workshops provide clear instructions and actionable steps to follow at home.
When patients adhere to these recommendations, noticeable improvements typically occur within 10–14 days.

Your first consultation includes a thorough review of your health history, lifestyle factors, and symptom patterns. This session can be completed virtually or in person at the clinic. You are encouraged to bring any recent labs or blood work, as this information helps create a more accurate and personalized treatment plan.
The Treatment
Your treatment plan is tailored specifically to your pain, anxiety, inflammation, and stress patterns. We use a blend of Chinese medical therapies, Functional Medicine assessment, and NET (Neuro-Emotional Technique).

The care plan is designed to be both effective and time-efficient. After your initial session, you’ll receive recommendations on the number of treatments needed, supplement guidance, Functional Medicine insights, lifestyle adjustments, and supportive NET strategies. You will also get a realistic overview of expected results.
